At Hunt Regional Healthcare, we make it easy and secure to access your medical records. Whether you’re the patient, an authorized representative, or an attorney, there are several convenient ways to request copies of your health information — including diagnostic imaging and billing records.
Patient Portal
Most of your health information can be viewed online through the Patient Portal. If you experience any issues accessing your portal, please contact us via the options listed on our patient portal webpage.
Request Your Records Online
Requesting your records from Hunt Regional Healthcare has never been easier. We’ve partnered with Verisma, an industry leader in the release of information field, for safe, secure medical record requests online.
Requests that meet all HIPAA and state requirements are typically fulfilled within 15 business days of submission.
Verisma’s secure online portal will guide you through every step — all you’ll need is access to a camera or smartphone to upload a photo ID.

If you’d like your records sent via a download link, Verisma will first verify your email address before sending it — be sure to check your spam or junk folder for their verification email.
Once your request is submitted, Verisma will email you a summary of your request along with a unique access code for secure tracking.

To request by mail or fax, complete the Authorization for Disclosure of Health Information Form (PDF), include a copy of a valid photo ID, and send both to:

Please be as specific as possible about the dates and type of records you need — this helps us process your request faster and more efficiently. Requests for a comprehensive history (“all records,” “all dates,” “unknown,” etc.) will be capped at ten (10) years of records.
Fees
Fees for copies of health care information are set in accordance with the Texas Health & Safety Code (THSC), Chapters 241, 311, and 324. The maximum allowable fees below are effective September 1, 2025. This schedule is provided as a courtesy; Hunt Regional Healthcare and Verisma are responsible for ensuring fees charged comply with the applicable THSC chapters referenced above.
